Waiver (please sign prior to arrival)

I release liability and I assume all risks for myself and/or my child whom ever will be participating at  "It's Slime Time" located at 646 Medford Ave Patchogue NY 11772 crafting activities.

I acknowledge that it is my responsibility to inform "It's Slime Time" of any type of allergies, skin conditions or medical conditions that myself or my child may have. Slime making may pose some minor risk of personal injury to individuals with allergies or sensitive skin. I understand that myself or my child should not play with slime if myself or my child is allergic to non-toxic school glue, borax, glycerin, glitter, food coloring, essential oils, latex, fragrance oils or any other additives that might be used in the making of slime and if myself or my child has an open cut or current skin rash. This product should not come in contact with face, eyes, mouth, or nose.

I understand and assume the risk for myself and my child. Through this document, I was made aware that none of the products sold by "It's Slime Time" are Consumable by mouth and therefor should not be eaten. None of the items sold at "It's Slime Time" at edible. It is my understanding that it is my responsibility to always inform and monitor my child to ensure that products by "It's Slime Time", including but not limited to slime and all slime ingredients shall not be consumed orally. I understand that product liability is transferred from "It's Slime Time", to myself and that I must continuously advise and monitor my child to ensure no oral or bodily consumption.

I understand and acknowledge that "It's Slime Time", is not responsible for any potential exposure to the Coronavirus/COVID-19 that myself, child and/or members of my party may risk. I also understand that no members of my party, myself, or my child are experiencing any symptoms of illness such as but not limited to fever, shortness of breath, chills, cough, muscle pain, headache, sore throat, new loss of taste or smell. I also acknowledge that I must comply with all set procedures to reduce the spread of COVID19 while attending slimeatory workshops and classes.

I confirm that my child is of age 3 or older. I understand that if my child is under the age of 3 they will be unable to participate in any"It's Slime Time", Slime Crafting activities.

While attending the class or the store I understand that myself, my child or anyone attending may be recorded and/or photographed. These recordings or photographs may be used on "It's Slime Time's", social media, websites and/or marketing efforts. It is my understanding that I must notify "It's Slime Time", if I do not wish for these images or media to be shared on any publications.
